2012 Summer Registration

 

 

 

 

 

Registration for the 2012 Outdoor DSP Season: 

 

All registration forms must be sent to our office location. Please attach all required forms with the correct cheque (made to OISC) and mail to the address below:

Forms could also be dropped off at our office location. Only during office hours (see below), will we accept them in person; otherwise there is a secure box outside the door where you can drop the form and cheque.

 

Tue: 11AM to 1PM

Thu: 4PM to 6PM

Sat: 9AM to 11AM *

 

* The office will be open from 9 AM to 4 PM from January to March to handle registrations.

 

Fees for the 2012 DSP summer season:

****Please note that all returned cheques will be charged an admin fee of $25. Any returned cheques must be replaced with a bank draft or a certified cheque within 7 days of the notice given to ensure your spot.

 

Refunds or Withdrawals:

 

• All requests must be submitted by mail or email to the DSP Administrator.
• Full refunds will be provided if we are not able to accommodate one of your three site selections and you do not want another site.    
• Prior to April 15^th, if you have received your site selection, an admin fee of $25 will be applied.
• After April 15^th but before the season starts a $75 admin fee will be applied.
• After the season has started, and prior to July 1^st, where a player suffers a season ending injury, a refund will be issued based on a prorating of time and less an administration fee of $75.
• No refunds will be issued after July 1^st.

DSP Registration Procedure:

Please Note:

• Players born in 2007 register U5
• Players born in 2005 and 2006 register U7
• Players born in 2003 and 2004 register U9
• Players born in 2001 and 2002 register U11

STEP #1: Select a location on the grid (Team Availability Grid ).

STEP #2: Select preferences; first, second and third choice (Mandatory).

STEP #3: Print and complete the registration form, all fields are required. (Registration Form).

 STEP #4: The OSA participation agreement. By signing the registration form you agree to this agreement. (Click here).

STEP #5: Mail the registration form with your payment. Please ensure that the form and accompanying documents are filled completely. Any incomplete forms may delay the registration process.  

Please make sure that you follow all the steps to ensure that your application is complete. Spaces are limited due to the field restrictions, so registrations will be processed on a first come, first serve basis. Any incomplete forms may result in delay of the registration process and as a result you might lose your spot.
