Registration for the 2012 Outdoor DSP Season: 

 

All registration forms must be sent to our office location. Please attach all required forms with the correct cheque (made to OISC) and mail to the address below:

Forms could also be dropped off at our office location. Only during office hours (see below), will we accept them in person; otherwise there is a secure box outside the door where you can drop the form and cheque.

 

Tue: 11AM to 1PM

Thu: 4PM to 6PM

Sat: 9AM to 11AM *

 

* The office will be open from 9 AM to 4 PM from January to March to handle registrations.

****Please note that all returned cheques will be charged an admin fee of $25. Any returned cheques must be replaced with a bank draft or a certified cheque within 7 days of the notice given to ensure your spot.

DSP Registration Procedure:

Please Note:

STEP #1: Select a location on the grid (Team Availability Grid ).

STEP #2: Select preferences; first, second and third choice (Mandatory).

STEP #3: Print and complete the registration form, all fields are required. (Registration Form).

 STEP #4: The OSA participation agreement. By signing the registration form you agree to this agreement. (Click here).

STEP #5: Mail the registration form with your payment. Please ensure that the form and accompanying documents are filled completely. Any incomplete forms may delay the registration process.  

Please make sure that you follow all the steps to ensure that your application is complete. Spaces are limited due to the field restrictions, so registrations will be processed on a first come, first serve basis. Any incomplete forms may result in delay of the registration process and as a result you might lose your spot.
